This article walks through the process of creating and editing a content request form, including the individual steps of our form-creation wizard.
The Divvy Details
One or multiple request forms can be created to help you manage a myriad of initiatives, content types, channels, etc. Each form can have up to 15 fields with 13 of them being user-defined fields.
To get started from the Content Requests Admin, click the Create New Form button in the upper right to initiate the form building process (a five-step wizard).

Step 1: Name your form and provide some initial instructions - This data will help requesters know they are filling out the right form and how to fill it out.
Step 2: Add Form Fields
A total of 15 fields can be added to a form. The first two fields (request title and requester's email) are required and not editable. However, you can edit the field names and placeholder text to help requesters complete these fields. For example, every request needs a title, but you may have some specific naming convention or format that you want requesters to use.
IMPORTANT NOTE: Field names have a max character limit of 60 characters. So keep your field names short/concise.
Beyond those first two fields, you have a lot of flexibility to build your forms however you see fit. You have 13 more fields to work with. Here are the types of fields you can include currently:
- Text fields - a single line of text
- Text areas - multiple lines/paragraphs of text
- Dropdown list - allows users to choose from a list of options
- File upload - allows users to upload up to 4 files per file upload field.
Pro Tip: Be thoughtful about the fields you include. Try to strike a balance to keep the form short (easy to submit), while also giving you the data you need to properly evaluate the request and run with it.
Step 3: Assign Form Administrators - These users can make edits to this form if needed in the future.
Step 4: Assign Request Moderators - Select users who should be notified when new requests are submitted, and should be able to review, edit, approve or decline requests.
Step 5: Set Notification Preferences
We've added a variety of notifications to not only help your team stay on top of incoming requests, but also to keep requesters updated on the status of their request(s). However, you may have your own communication preferences. So we've given you options. You can choose between the following:
- Send status notifications automatically - This default setting will keep requesters updated automatically
- Let me choose which to send - Upon a status change, this option lets you decide whether or not to send a notification to the requester. You can also add a custom message if desired.
- Don't send status notifications - We silence all notifications
FINAL STEP: Click Create Request Form!
With your new form created, you'll be able to see the fruits of your labor by viewing the full detail view of your form. This is also where you can make changes to your form in the future.
How to Edit an Existing Request Form
Should you need to make a change to your form, you'll want to navigate to the form detail view (shown above). Here's how:
- Head back to the Content Requests Admin (top right menu, click your name)
- Click the pencil icon next to the form you want to edit
From the form detail view, you can click Edit Details and make any necessary changes to your fields, including setting it as active or inactive, reordering the fields via drag/drop, and adding fields.
IMPORTANT NOTE: Any changes you make to your form will not retroactively change any existing requests that have already been submitted. Existing requests will still maintain the field structure that was in place when they were submitted. Any new request submitted after making your changes will use your new version.
